CO-18 on a dental claim: duplicate denials, and when to check the original
CO-18: Exact duplicate claim or service. What it means on a dental claim, why dental offices get it, what to do, and whether the patient owes it.
By Dr. Salem Sayegh, general dentist, Glendora, California ·
The short answer: CO-18 means the payer already received the same claim or service and denied this one as a duplicate. Usually it is harmless: the claim was sent twice, or sent electronically and on paper. The thing to check is that the original was actually paid, and that this was not a second, legitimate service on the same day.
| Code | CO-18 (adjustment reason code) |
|---|---|
| Group | CO, contractual obligation |
| Meaning | Duplicate of a claim or service the payer already has |
| Is it a denial? | Yes, for the duplicate |
| Bill the patient? | No |
| Usually fixable? | Usually nothing to fix; check the original |
Why dental offices get CO-18
- The claim was resubmitted while the original was still processing.
- The same claim went electronically and on paper.
- A corrected claim was sent as a new claim instead of a correction.
- Two genuine services on the same tooth and date, such as two separate radiographs, that look identical to the payer.
What to do about CO-18
- Find the original claim and confirm it was processed and paid.
- If the original was paid, post nothing for the duplicate beyond closing it.
- If the original is missing or denied, call the payer before resubmitting, or you will get another CO-18.
- If the two services were genuinely separate, resubmit as a corrected claim with a note explaining why.
Can you bill the patient for CO-18?
No. A duplicate denial creates no new patient balance.

Common questions
Should I resubmit after a CO-18?
Only after confirming the original was not paid. Resubmitting a claim the payer already has just produces another CO-18.
Why did I get CO-18 on a corrected claim?
The correction was probably sent as a new claim. Most payers need corrected claims marked as replacements of the original, with the original claim number.
